Palliative Care aims to help you have a better quality of life during the final 6-12 months of your life. High Quality Palliative Care can help manage a person’s pain and end of life symptoms, provide personal care, support families and provide emotional, spiritual and psychological support.
In-home Palliative Care Nursing
Our Palliative Care nurses support you and your family to manage the physical, emotional, social, cultural and spiritual aspects of a person’s death with skill, dignity and respect.
Our Highly qualified Palliative Care Nurse Practitioner leads our Palliative Client’s care in consultation with your Health Care Team and include your family and loved ones, supporting them through the all aspects of care and support.

IN-HOME 24/7 NURSING
Our Palliative Care Trained Nursing and Lifestyle Assistant Team can provide 24/7 support to provide you with an individualized plan to best meet your needs during this difficult time.

MEDICATION MANAGEMENT
Our Palliative Care Team can help you manage your medications to enable you to remain independent as long as possible, taking the right doses at the right time and effectively assist your family manage them when you can no longer. (we need to discuss it this way to meet NDIS goals and funding)

SYRINGE PUMPS
Our Palliative Care Nurses are trained in management of subcutaneous medication management when you need it. A syringe pump is used to provide a constant infusion of medications such as pain medication when a person can no longer swallow.

END OF LIFE MANAGEMENT
Our Palliative Care Team, lead by our Palliative Care Nurse Practitioner can help with the emotional, cultural and spiritual factors of your care, and provide support for your loved ones.

We find out what it means to you and respect your and your Family’s wishes, aiming to relieve your pain and ease your symptoms so that you can pass in your own home if that is your preference.
What is In-home Palliative Care?
When you are living with a life limiting diagnosis, you may make the choice to spend your remaining time at home surrounded by your loved ones.
In-home palliative care aims to support your quality of life and ensure that you are receiving care and support , whilst keeping you as comfortable as possible in your own home. Depending on your specific needs and condition, you may receive in-home care for days, weeks or months
Support for Carers
Caring for and supporting someone approaching the end of their life can be emotionally draining and physically demanding for your family and loved ones who have been caring for you . In-home palliative care provides support and respite for these carers.
With well educated and skilled staff members who understand your loved one’s needs and changing condition, it allows you to take the time to care for yourself, such as going out for a meal, meeting a friend or going for a walk.
We also offer respite services, which give you the chance to have an uninterrupted night’s sleep as we come and stay overnight, looking after all of your usual tasks and caring for your loved one.
Our respite services are available at short notice if you suddenly need a rest, however we recommend you plan your respite services so that you are given a regular break.
How do you access In-home Palliative Care?
There are several ways you can access our in-home palliative care, depending on your age and situation.

My Aged Care
If you are eligible for aged care services through My Aged Care, you may be able to use government funding for some or all of your care.

NDIS Plan
If you are under 65 and eligible for NDIS services, you may be able to allocate some of your funding to palliative care services.

DVA Community Nursing
If you are a veteran and are eligible for DVA funding and assistance, you may be able to access our services using the community nursing funding.

Private Funding
If you are not eligible for government funding, or if you require extra assistance than what your funding covers, you can access our services as a private client.
