What is In-home care?
In-home care is when trained carers come into your home to provide support and services that allow you to live at home
as independently as possible. In-home care is provided to keep you safe in your home, well and independent, and connected to your community.
At Focused Health Care, we provide support with things like personal care, cleaning, cooking or gardening. Whatever you or your loved one needs, we’re here for you. We’re run by Registered Nurses so we are both deeply caring and have the clinical background to provide more complex support if you need it.
What does an In-home care package offer my family?
Each in-home aged care package is unique to your needs.
The supports and services that are included in each in-home aged care package will vary according to your circumstances.

We’re Registered Nurses
Focused Health Care is operated by Registered Nurses. We have extensive experience in providing in-home care; we understand your needs from both a care and clinical perspective. We can provide a range of services from simple to complex, including in home nursing care or palliative care nursing.
We’re accredited, experienced and professional
As a registered Aged Care and NDIS provider, we meet government safety and quality standards. That means when we provide in-home nursing care we provide carers who are experienced, flexible and respectful.
We’re always here for you
We understand that you don’t have time to sit on a wait list, so we don’t have them. You can pick up the phone and chat to us (a real person, not a call centre) whenever you need help – that’s 24 hours a day, seven days a week.
How do I access In-home care?
There are many ways you can access in-home care services. The method of access and funding will depend on your age, needs and specific situations.
If you are over 65, you may be eligible to access in-home care using My Aged Care funding. The amount of funding available will depend on your financial situation and your specific needs.
Contact My Aged Care on 1800 200 422. They can provide support to help you access Australian Government-funded aged care services.
You will then need to participate in an Assessment to determine the level of support you need. Focused Health Care can be with you for this assessment.
My Aged Care will then send you an approval letter.
My Aged Care will then place you on the national waiting list for allocation of a Home Care Package.
When you receive a Home Care Package letter contact Focused Health Care and we can begin to set up the services and care you need.
If you are living with a disability that impacts your ability to live independently, you may be eligible for NDIS funding for your in-home care. The amount you are provided will depend on your specific disability.
Make an Access Request through the NDIS by calling 1800 800 110. If you are a person with a disability and would like to join the NDIS, you will need to meet its Access Requirements.
You’ll meet with an NDIS representative who’ll ask you questions about your daily life to help them work out the types of support and the amount of funding you’ll receive.
The NDIA will then send you a letter telling you whether or not you’ve been approved for NDIS funding.
When you receive your approval letter from the NDIA contact Focused Health Care and we can begin to set up the services and care you need in collaboration with your NDIS Local Area Coordinator.
If you are a veteran eligible for DVA funding, you may be able to use this to cover some or all of your in-home care. You will need to assess your eligibility with the DVA.
If you are not eligible for government funding, or if it does not cover the entirety of your costs, you can receive in-home care as a private client, using your private funds. Some health funds may also cover part of the cost, depending on your level of cover.
