Our nursing team consists of a Nurse Practitioner and Registered Nurses, all of whom are fully qualified and experienced.
Our Nurse Practitioner Services
Nurse Practitioners are licensed practitioners who obtain graduate education at the Masters level and beyond to obtain national board certification.
Our Nurse Practitioner’s scope of practice is in Palliative Care, Aged Care and Terminal chronic conditions.
How Our Nurse Practitioner Assists Our Clients
The service our Nurse Practitioner offers includes:
- Diagnosing and treating a wide range of health conditions
- Designing and implementing therapeutic regimens
- Initiating referrals to other health professionals as required
- Ordering and interpreting pathology and radiology tests
- Prescribing medication or deprescribe
- Accessing the Pharmaceutical Benefits Scheme
- Providing education and counselling clients
The benefits of having a Nurse Practitioner on our team
Our Nurse Practitioner provides support and advice for families and clients requiring palliative care.
She conducts home visits and provides symptom and pain management plans, with the focus on helping clients remain at home if that is their wish.
This service can be provided whether an illness is potentially curable, chronic or life-threatening and is appropriate for clients with non-cancer diagnoses as well.
She also works collaboratively with other health professionals and in conjunction with GPs, specialists or other nursing services to ensure continuity of care during this difficult phase.

These services require a high degree of skill – that’s why we rely on Registered Nurses, not care workers.
Registered Nurses are highly trained professionals who have completed a degree in nursing. Our nursing skills mean you can receive professional care at home. You’re more at ease in your own space and you’re at lower risk of infections, falls or medication errors than in hospital.
ACCESSING SERVICES
The way to access our in-home care varies a little depending on where your funding comes from.
We’re happy to help you through the process of getting government funding through My Aged Care or the NDIS as it can be confusing at times – give us a call on 1300 201 351 or 0499 487 728.
If you’re coming as a DVA Community Nursing client, you’ll need a referral. If you’re self-funded, simply give us a call and we’ll start developing an in-home care package that meets your needs.
SWITCHING PROVIDERS
You have choice and control over who comes into your home. That means you’re able to end any agreement with your existing service provider and join us. We’d love to work with you and show you the Focused Health Care difference. We can liaise with your old provider for a handover and to sort out any financial matters.
If you’re not happy with our services and would like to switch from us to another provider, then we ask for two weeks’ written notice. We don’t charge exit fees – we want you to be happy with your care and we don’t charge you for exercising your right to choose.
