Restorative care is designed to help you regain strength, confidence and independence at home after illness, injury, or a hospital stay.

Founded and directed by Registered Nurses, Focused Health Care delivers nursing, allied health and in-home supports to help you feel more like yourself again.

What is the Restorative Care Pathway?

The Restorative Care Pathway is part of the Support at Home program under MyAgedCare. It is a short-term, goal-focused pathway designed to help older people regain or maintain function and delay the need for long-term care.

Various supports are available, depending on your needs. It may include nursing care, allied health, support with daily activities, equipment, or minor changes around the home.

Restorative care may suit older people who have:

  • returned home after a hospital stay
  • experienced a recent fall
  • noticed reduced mobility, balance or strength
  • found everyday tasks more difficult than before
  • had a change in care arrangements at home
  • been diagnosed with a condition affecting day-to-day function.

Frequently asked questions

If you are approved for the Restorative Care Pathway, you receive a support plan and budget for a 16-week period. In some cases, you may be eligible for up to 2 episodes of restorative care in the same year.

The Restorative Care Pathway sits within the Support at Home program. Clinical supports are fully funded by the government but a participant contribution may apply to independence or everyday living supports depending on My Aged Care Assessment.

We can also discuss privately funded options with you if you are not eligible for Support at Home, need support sooner, or want to add services beyond your funding.

To access the Restorative Care Pathway, you need an aged care assessment. If restorative care is considered appropriate, your assessor will work with you to set goals and develop a support plan.

Once approved, Focused Health Care can work with you to deliver restorative care services in your home, in the community, or across both settings, depending on your goals and support plan.
