Support at home

Independence supports

When everyday routines become harder to manage, the right support at home can make a meaningful difference.

Founded and directed by Registered Nurses, Focused Health Care provides independence supports that help people maintain daily routines, stay connected to their community and continue living at home with confidence.

Independence supports at home for daily routines and connection

Our independence supports are designed to assist people at different stages of life and care, including:

  • Older people receiving services through the Support at Home program
  • NDIS participants requiring help with daily living and community participation
  • Veterans accessing eligible support through DVA
  • Individuals and families seeking privately funded care without eligibility requirements
  • People who need flexible support to remain independent at home

Independence supports under the Support at Home program

Under the Support at Home program, Independence Supports sit alongside Clinical Supports and Everyday Living Supports as part of the new service structure.

Focused Health Care can act as your single provider, helping coordinate the support you need through one team. You will also have a dedicated care partner to help plan, review and adjust your services as your needs change.

Our team takes the time to understand your preferences, your goals and the way you want your support delivered. We know that consistency, respect and clear communication matter just as much as the service itself.

Support dignity and independence at home
Help maintain daily routines and familiar activities
Reduce stress for individuals and families
care
Provide reliable support that can adapt as needs change
Independence supports for NDIS participants

Independence supports for NDIS participants

As a Registered NDIS Provider, Focused Health Care supports participants to access practical services that align with their goals and daily living needs.

Depending on the participant’s plan, these supports may relate to:

  • Assistance with daily life
  • Social and community participation
  • Transport supports where applicable

We work collaboratively with participants, families and support coordinators to provide services that respect choice, control and continuity of care.

Private independence supports

If you need immediate support or do not meet the eligibility criteria for government-funded services, private care may be an option.

Our privately funded independence supports offer:

  • Flexible scheduling
  • No eligibility assessments
  • Direct access to experienced, professional support staff
  • Support that can be arranged alongside existing funded services or on its own

This pathway may suit individuals or families looking for prompt, flexible help with personal care, respite, transport or daily routines at home.

Independence supports

Independence supports for veterans

For eligible veterans, some in-home supports may be available through DVA funding pathways.

Depending on eligibility and approved services, support may include:

Personal care
Medication support with routines and prompting
Private-nursing
Respite care
Other practical assistance delivered at home

Our team can help you understand the available pathway and discuss the type of support that may suit your circumstances.
